forms.team is an AI-powered form builder. Create surveys, intake forms, registrations, and questionnaires in minutes — then collect responses, share via link or embed, and analyze results in real time.
AI assistant
The assistant can build complete forms from a description, rewrite questions, suggest fields, restructure your form, and add embeds (videos, maps). Tell it what you want in plain English and it edits the form live.
Fields & embeds
Short/long text, email, phone, number, multiple choice, checkboxes, dropdowns, ratings, scales, date/time, file upload, signature, plus layout fields like headings, paragraphs, dividers, and embeds.
Sharing & responses
Every published form gets a clean public URL like forms.team/f/your-slug. You can also embed forms on your site or share via QR code.
Customization
Yes — pick a color theme and light/dark background per form. Pro users can replace the forms.team logo with their own brand.
Account, billing & security
Go to Settings in your dashboard and choose Upgrade. Billing is handled securely via Stripe.
